VERRUCA PLANA
Verrucae planae are sometimes missed because the pathology is not very dramatic. Hyperkeratosis with 'holes' overlies a zone of hypergranulosis.  The epidermal surface has an undulating 'hill and dell' contour rather than the jagged 'mountain and valley' contour of a verruca vulgaris. The superficial keratinocytes, particularly those in the dells, have clear cytoplasm, and the nuclei are often centered within the cells.

Holes in the hyperkeratotic layer are shown, and hypergranulosis is illustrated. Cells having a nucleus centered within clear cytoplasm occupy the superficial one-fourth or more of the epidermis.
