Metaplastic or Sarcomatoid Carcinoma

The tumor cells in this case were S100 negative and CD68 negative. The fact that the cells are CD68-negative in this case would argue against the possibility that this might be a collision tumor (mixture of AFX and SCC).

Vimentin:
The spindle cells are uniformly postives for vimentin. The spindle cells are uniformly negative for keratin markers (AE1/AE3).

AE1/AE3 (keratin):
Many of the epithelial cells are strongly positive. There is a gradation in positivity with some of the epithelial cells being negative.

Vimentin:
The epithelial cells in the nests in the top part of the picture are negative. There are cells having transitional morphology that are variably positive in the lower part of the picture.
