IDIOPATHIC SCROTAL CALCINOSIS
Scrotal nodules composed of calcium without evidence of a disturbance in calcium or phosphorus metabolism constitute idiopathic scrotal calcinosis. This condition is described under calcinosis cutis in most textbooks. The calcium deposits are sometimes associated with a granulomatous reaction, but there may be deposits that are not associated with an inflammatory reaction. A vulvar counterpart has been described, and there have been a few cases associated with epithelial cysts.

A nodule from the scrotum of a 21 year old male:
 
Scanning power view. Multiple collections of basophilic material (calcium) are found in the dermis. The hard nature of some of the deposits has resulted in the ripping of the tissue during microtomy.
A medium power view of above. Small calcospherites are associated with fibrosis but little cellular reaction.
Another medium power view. Finely granular calcium salts (top of picture) are associated with a granulomatous reaction.
