FIBROFOLLICULOMA

A TYPE OF FIBROEPITHELIAL FOLLICULAR HAMARTOMA
Solitary tumor from the left ear of a 40 year old male.

Arcuate, thin strands of epithelium that occasionally interconnect distinguish this type of fibroepithelial follicular hamartoma from other variants. Most of the epithelium is mature, but a few small foci of more primordial follicular epithelium do not negate the diagnosis.

Scan power view. The main part of the tumor is  between the two arrows. There is a smaller lobule to the right of the right-most arrow. The hyperplastic fibrous sheath forms an integral part of the tumor. There are several interconnecting, arcuate, delicate strands of epithelium that are characteristic of fibrofolliculomas.
Low power view near the base. Interconnecting delicate strands of epithelium are separated by dense stroma of follicular fibrous sheath type. 
High power view of above. There is a mitosis (MIT), but this is the only one I found. The epithelium in this field is mature. This epithelium can be variously interpreted as mantle type epithelium or as isthmic type epithelium.
Another high power view near the base. The epithelium and stroma are primordial but should not be interpreted as basal cell carcinoma in this setting. Trichogenic foci can be found in variants of this type of tumor.
Medium power view showing the epidermal surface above part of the tumor. If it were not for the distinctive connective tissue component, this field could have been found in a tumor of the follicular infundibulum.
