CUTANEOUS CRYPTOCOCCOSIS

This is from a raised, ulcerated leg lesion from an elderly diabetic male who is a renal transplant recipient. The cerebrospinal fluid was normal as was the chest roentgengram.
 
(intermed mag): Infiltrate of activated histiocytes. Round and oval defects contain faintly basophilic organisms that vary in size. Organisms tend to
marginate at the periphery of defects.
mixed infiltrate with pale staining organisms. Individual organisms separated from neighbor inflammatory cells by symmetrically rounded clear space.
Organisms centrally are in cytoplasm of giant cell.
silver methenamine stain showing numerous organisms
silver methenamine stain with loosely spaced organisms all outlined by rounded, clear spaces. Organisms with narrow-based buds are represented. Some
of the organisms are concave along one surface.
mucicarmine stain. The capsule of the organisms is mucicarminophilic. This is characteristic but not diagnostic.
